IMPORTANT NOTE: If you are a divorced person or a party in a divorce action seeking to resume a prior surname, a separate simpler procedure applies. Typically, one can change their name as a result of the divorce if they included this request in their pleading. If one does not include such request in their divorce pleading then they would have to go through a separate procedure. Nonetheless, our product/service should not be used by individuals seeking to resume a prior surname prior to or subsequent to the entry of the divorce decree. The Prothonotary's Office or the clerk of the court in your county should have information relating to this particular procedure.
PLEASE NOTE: The Allegheny County courts recommend a specific paper-format of Pennsylvania name change documents, which are offered locally. Our products, though designed to meet state legal requirements, are sometimes denied by the clerks solely because Petitioners are not filing these "recommended forms". Other customers of ours have prepared their documents using our products, submitted them to the Allegheny Court clerk, and have never received a complaint or issue from the court. As such, while our staff has not located any significant differences in content or language, our product has been randomly denied. We have not been able to successfully remedy this issue and therefore want to alert our visitors and customers to the potential problem. You may wish to contact your local court clerk or Prothonotary's office to learn more about the local forms and filing procedures for Allegheny County.

PLEASE ALSO REMEMBER: When the change of a name is for a minor, the application for a name change must be brought by one or both of the minor’s parents, but if the minor has two living parents and one does not join in the application, that non-petitioner parent must be served with reasonable notice of the application and, if that parent objects, a hearing will be held to determine whether the change of name is in the best interest of the minor. If you anticipate that the non-petitioner parent will object and/or if the non-petitioner parent does object our product should not be used and a registered attorney in your area should be consulted.
