Once you have a divorce judgment, you may resume your maiden name through the Clerk’s Office for a $5.00 fee. The resumption of maiden name forms should be located in the Civil Division of the Clerk’s Office in your local court. You will need a copy of the divorce or death certificate to attach to the application forms. To resume a prior name, you will need to fill out the application forms and bring a certified copy of the divorce or death certificate.
