Contact the Office of Vital Records in the state where the minor was born, for information about name changes that can be obtained without going to court. Additionally, persons who have been convicted of a felony, have claimed bankruptcy, or have a lien or financial judgment filed against them should not use this product. In such cases, a licensed attorney should be contacted. During a proposed name change for a minor, the petition must be brought by both of the minor’s parents, or by one parent with the Notarized consent of the other. All adults who retain legal rights over the child must be given proper notice of the proposed action. Courts typically conduct hearings to determine whether the change of name is in the best interest of the minor and hear any objections which may be raised by interested parties. If you anticipate objections, our product should not be used. In such cases, consult a licensed attorney in your area. NOTE: You may only use this Kit if both parents consent to the name change. If one parent does not consent, you MUST seek the advice of an attorney. This Kit is not appropriate for this type of situation.
