IMPORTANT NOTE: This Kit is NOT designed to be used in Hennepin County (Minnesota). A separate name change product will soon be available for use in Hennepin County. This product is intended for use in simple, uncontested actions only. Our products do not address the following situations: Persons seeking a name change for a minor due to adoption. Typically a simplified procedure is available. Similarly, if the minor is under the age of one year OR if you are only correcting a first or middle name, you MAY not need to go through the court-ordered process. Contact the Office of Vital Records in the state where the minor was born, for information about name changes that can be obtained without going to court. Additionally, persons who have been convicted of a felony, have claimed bankruptcy, or have a lien or financial judgment filed against them should not use this product. In such cases, a licensed attorney should be contacted. During a proposed name change for a minor, the petition must be brought by both of the minor’s parents, or by one parent with the Notarized consent of the other. All adults who retain legal rights over the child must be given proper notice of the proposed action. Courts typically conduct hearings to determine whether the change of name is in the best interest of the minor and hear any objections which may be raised by interested parties. If you anticipate objections, our product should not be used. In such cases, consult a licensed attorney in your area.
