Contact the Office of Vital Records in the state where you were born, for information about name changes that can be obtained without going to court. Additionally, persons who have been convicted of a felony, have claimed bankruptcy, or have a lien or financial judgment filed against them should not use this product. In such cases, a licensed attorney should be contacted.
