This product is intended for use in simple, uncontested actions only. Our products do not address the following situations: Persons seeking a name change due to marriage. Persons seeking a name change while divorcing. Typically simplified procedures are available. Similarly, if you are only correcting a first or middle name, you MAY not need to go through the court-ordered process. Contact the Office of Vital Records in the state where you were born, for information about name changes that can be obtained without going to court. Additionally, persons who have been convicted of a felony, have claimed bankruptcy, or have a lien or financial judgment filed against them should not use this product. In such cases, a licensed attorney should be contacted.

IMPORTANT NOTE: In Connecticut name change actions are heard in Probate and Superior Courts. This product should ONLY be used when filing in Probate Court. We do not currently offer a product for filing a Connecticut adult name change action in Superior Court. Consequently, if you anticipate filing your name change action in Superior Court, do not purchase this product. We recommend you contact your local Superior Court for further information. In keeping with the above, name changes which are sought due to divorce or dissolution of marriage in Connecticut will only be considered in Superior Court.
